Enquiry Form (#1)

Often there's some confusion about whether the bridal couple is making bookings in the surname of the bride or groom, and the person making the enquiry might be a different person altogether.

The Booking Name recommended is the one used by the couple for all other wedding-related bookings (venue, florist, etc)   See the note below.

If you are enquiring for someone else, please make the Booking Surname relate to the bridal couple, and the other contact details (Phone, email, etc) relate to yourself making the enquiry.

You can still fill out the Wedding Dance Checklist, if you wish.

The Booking Name recommended is the one used by the couple for all other wedding-related bookings (venue, florist, etc).   Good examples of a booking name are:

  1. Jane Smith and Tony Brown - September 2021
  2. Jane (Smith) and Tony Brown - September 2021

The second example indicates the bride is changing her surname - which is less common now.

Write the names as they would wish to be called or written to.  This subtle guidance helps us get the couple's name right on subsequent congratulations cards.

Including the wedding month is also a useful touch.

Details of person making the enquiry.  We will contact this person.

This is a checklist of questions we ask you during our complimentary lesson and chat.  It's a good idea to consider these before your first appointment.  From your answers, we make certain recommendations and tailor our teaching to suit what you wish.

It's  useful even to just partially complete this checklist, and it may get you thinking about aspects you had not considered (such as a group lesson for your bridal party).

Just fill out whatever you can.  None of the following fields are compulsory to complete.

Lessons are available almost any time of the day and on most days - if booked well in advance.

Bridal party, family, and guests often arrive just before the wedding.  We can work around this, if booked well in advance.

If the song is to be played by a live band/artist, it's a good idea to coordinate a practice session.

If the song is to be played from a recording, it's a good idea to be very sure who is responsible for ensuring the song is available at the venue.

We also need to check the necessary equipment can play the song, so we need to coordinate with the venue or DJ.

We also need to know who will start the music and upon what signal from the bridal couple.

Dress and Shoes

Fill out whatever you can.

After having gone through this checklist, you may have some further questions.  Please add them to your Message field above.
